2. Information We Collect and Protect

To ensure your safety and deliver results that look beautiful for years, we collect the following PHI:

• Medical History: Information regarding skin conditions, allergies, current medications, and previous cosmetic procedures (e.g., prior PMU that may require
  Cover-Up/Correction).
• Treatment Records: Detailed notes on pigment colors used, needle configurations, skin undertones, and tissue response during procedures like
  Nano Brows, Ombré Brows, Lip Blush, or Hollywood Carbon Facials.
• Clinical Photography: High-resolution before-and-after photos used exclusively for medical documentation and treatment planning. (Photos are only used for marketing or portfolio purposes if you have signed a separate, explicit media release form).

3. How We Use Your Health Information

Your PHI is used exclusively to provide safe, evidence-based care tailored to your specific skin type, as well as to facilitate payment and manage our internal operations. Specifically, we use this information for:

• Treatment: To assess your medical suitability for permanent makeup or laser treatments; predict how pigments will heal based on your cellular skin science
  and undertones; recognize and manage any potential contraindications or adverse reactions; and develop evidence-based aftercare protocols tailored
  specifically to your skin type and procedure.
• Payment: To process payments for the services we provide to you. This may include sharing necessary information with our payment processing partners (such as Klarna, Affirm, Afterpay, or Zelle) to complete transactions.
• Health Care Operations: To support our daily business operations, such as quality assessment, training, and utilizing our secure, HIPAA-compliant CRM platform (Ascendea) to manage your records and appointments.
• Business Associates: We may share your PHI with trusted third-party vendors (Business Associates) who provide services on our behalf, such as our CRM platform or appointment booking software. These Business Associates are legally bound by a Business Associate Agreement (BAA) to protect your information with the same rigorous standards we apply.
• Authorization Required for Other Uses: Any uses or disclosures of your PHI not described in this Notice will be made only with your written authorization, which you may revoke at any time.

4. Your Rights Regarding Your PHI

Under our HIPAA compliance framework, you have the right to:

• Access Your Records: Request a copy of your medical history, treatment notes, and clinical photographs.
• Request Corrections: Ask us to amend any inaccurate or incomplete health information in your file.
• Confidential Communications: Request that we communicate with you about your treatments in a specific manner or at a specific location to ensure your
  privacy.
• Right to an Accounting of Disclosures: You have the right to request a list of certain disclosures we have made of your health information for purposes other than treatment, payment, or health care operations.
• Revoke Consent: Withdraw your consent for the use of your clinical photographs for marketing purposes at any time.

We are required by law to maintain the privacy of your PHI and to notify you promptly if a breach occurs that may have compromised the privacy or security of your unsecured information.
